Medicare GLP-1 Bridge Program 2026:
Medicare GLP-1 Bridge Program: What Patients Need to Know
Starting July 1, 2026, Medicare will provide access to certain GLP-1 medications for eligible patients with Medicare Part D coverage through the new Medicare GLP-1 Bridge Program.
This program may help qualifying Medicare patients access certain GLP-1 medications for weight management with a $50 monthly copay. Covered medications listed by Medicare include Foundayo, Wegovy, and Zepbound KwikPen.
For many patients, this is an important update. Until now, Medicare coverage for weight loss medications has been limited, leaving many patients to pay out of pocket or go without treatment.
At Elless, we are helping Louisiana residents understand the new Medicare GLP-1 Bridge Program, review eligibility, and prepare documentation for prior authorization when appropriate.
What Is the Medicare GLP-1 Bridge Program?
The Medicare GLP-1 Bridge Program is a temporary program that allows eligible Medicare Part D patients to access certain GLP-1 medications for weight management.
GLP-1 medications work by helping regulate appetite, fullness, and food intake. These medications are most effective when used alongside a long-term lifestyle plan that includes nutrition, movement, and ongoing medical follow-up.
The Bridge Program is not automatic coverage for everyone. Patients must meet Medicare’s eligibility criteria, and prior authorization may still be required.
When Does Medicare GLP-1 Coverage Start?
The Medicare GLP-1 Bridge Program begins July 1, 2026.
Patients who are interested should begin preparing early by reviewing their:
Current height and weight
BMI
Medicare Part D coverage
Weight-related health conditions
Prior medication history
Previous GLP-1 use, if applicable
Having this information available may help make the prior authorization process smoother.
Who Qualifies for the Medicare GLP-1 Bridge Program?
To qualify for the Medicare GLP-1 Bridge Program, patients must generally:
Be 18 years or older
Have Medicare Part D drug coverage
Be prescribed a covered GLP-1 medication for weight management
Meet Medicare’s BMI and medical diagnosis criteria
Use the medication as part of a lifestyle program focused on diet and exercise
Eligibility is based on Medicare’s criteria and may depend on the patient’s BMI at the time GLP-1 therapy is started.
BMI 35 Weight Chart: What Weight Equals a BMI of 35?
One Medicare GLP-1 Bridge Program eligibility pathway is having a BMI of 35 or higher.
Many patients find it easier to understand BMI by looking at height and weight. The chart below shows the approximate weight where BMI reaches 35.
Height | Approximate Weight at BMI 35 |
5'0" | 179 lb |
5'1" | 185 lb |
5'2" | 191 lb |
5'3" | 198 lb |
5'4" | 204 lb |
5'5" | 210 lb |
5'6" | 217 lb |
5'7" | 224 lb |
5'8" | 230 lb |
5'9" | 237 lb |
5'10" | 244 lb |
5'11" | 251 lb |
6'0" | 258 lb |
If your weight is at or above the number listed for your height, your BMI is approximately 35 or higher.
This chart is a quick reference only. Your actual BMI should be calculated using your current height and weight during your medical visit.
Can You Qualify With a BMI Below 35?
Yes, some patients may qualify with a BMI below 35 if they also have certain qualifying medical conditions.
Patients with a BMI between 30 and 34.99 may qualify if they have at least one qualifying condition, such as:
Heart failure with preserved ejection fraction, also called HFpEF
Uncontrolled high blood pressure
Chronic kidney disease stage 3a or higher
Prediabetes
Prior heart attack
Prior stroke
Symptomatic peripheral artery disease
Patients with a BMI between 27 and 29.99 may qualify if they have at least one of the following:
Prediabetes
Prior heart attack
Prior stroke
Symptomatic peripheral artery disease
What GLP-1 Medications Are Covered?
Medicare lists the following medications under the Medicare GLP-1 Bridge Program beginning July 1, 2026:
Foundayo® tablet
Wegovy® injection or tablet
Zepbound® KwikPen only
Coverage may still depend on eligibility, prescription requirements, pharmacy processing, and prior authorization approval.
Is Prior Authorization Required?
Yes. Prior authorization may be required for the Medicare GLP-1 Bridge Program.
This means your provider may need to submit documentation showing that you meet Medicare’s criteria. Documentation may include:
Height and weight
BMI
Diagnosis history
Medication history
Weight-related medical conditions
Lifestyle program participation
Current Medicare Part D information
Submitting a prior authorization does not guarantee approval. Coverage depends on Medicare Bridge Program rules, your Part D plan, and processing requirements.
Who May Not Qualify for the Bridge Program?
Not every Medicare patient will qualify for the Medicare GLP-1 Bridge Program.
Patients may not qualify if they already receive GLP-1 coverage through their Medicare Part D plan.
Medicare also notes that patients with type 2 diabetes, moderate-to-severe sleep apnea, or fatty liver disease may not be eligible for the Bridge Program itself, although their Part D plan may have a different coverage pathway.
This is why it is important to review your diagnosis, medication history, and plan details before assuming which coverage route applies.
How Much Does the Medicare GLP-1 Bridge Program Cost?
For eligible patients, Medicare lists a $50 monthly copay for covered GLP-1 medications through the Bridge Program.
This copay refers to the medication cost under the program. It does not include provider visit fees, clinical evaluation, or administrative fees for prior authorization submission.
